There are a number of factors that come to play in creating one of the most challenging complex human endeavors. Through software we turn abstraction and desire into the actual and the current. And buried in our code are the complex synergies of product management, architecture, design, engineering, business, and legacy. All this created by humans, and fixed by humans. To understand just what is going on to the customers due to our products' engineering, or how best to re-engineer and manage is more complex than we give credit to.
